Please address your referral to a named specialist, and include your signature, provider number and date of referral.

To ensure that we are able to provide the best service that we can, please include the reason for the referral, and all relevant medical history and investigation results.

Please ensure we receive your referral prior to the date of your patient’s first appointment with us.

Referrals will be actioned within 1 business day and patient bookings made as appropriate.
